If you have artificial joints, we recommend that you take premedication for all dental appointments for the first two years after the most recent joint was placed. The choice is ultimately yours, but you must fill out and sign this consent form in either case. More information can be found here and here.

In order to transfer your x-rays from another dentist to our office, you must fill out this form and we can send it to your previous dentist.
